How to build ground anchor covers for indoor wall - Advice requested

In our climbing wall we have a concrete floor with unrated eye bolts with 75mm thick rubber crumb tiles. These currently have a 50mm x 140mm cut out to make the eye bolt accessible, however, we are looking to make removable covers so they are less of a trip hazard. Does anyone have any experience or advice on this?

Our old wall had very small holes no bigger than 80/80mm across and either a link of chain or some maillons you had to pull out to clip a carabiner onto. Never saw anyone trip over the small holes although I think originally you pulled out the rubber plug to access the maillon but these obviously went missing or was not needed. 

Are people tripping over the holes you currently have?

I usually use the bit iv drilled out.

If you have some spare rubber crumb, get a hole drill cut some pucks, trim a bit off the bottom so they sit flush on top of the anchor.

Drill a small hole and thread a bit of cord through to pull them out when needed.

If you don't have rubber crumb I have seen centres do this with wood also.

Why not cover them over and have slings through them which you can clip into.
