Was it an automated camera attached to the ISS? Or taken by someone who was docking or on a spacewalk?

Astonishing.

Why did they hold the camera upside down?

I think there are cameras attached to the vehicle with all sorts of vantage points , as the HDEV pics show (when they're working)

On a slightly different topic I had a chance view of two satellites chasing each other right overhead  around midnight last night. Googling suggests they were probably TanDEM-X and TerraSAR-X, two German mapping satellites who work together to produce images. I estimated about three degrees of separation : apparently they can be as close together as a few hundred metres or as far apart as six or seven kilometres.  Great to watch and a pleasing fluke observation, though I know at least one website which will have predicted sighting times.
